APCA Needs Survey ($20 Reward)

YOUR FEEDBACK WILL HELP OUR COMMUNITY LEARN ABOUT DIFFERENT HEALTH AND LIFESTYLE-RELATED FACTORS AFFECTING THE HEALTH OF ASIAN AMERICANS, NATIVE HAWAIIAN, AND PACIFIC ISLANDERS.

*The first 100 participants who fully complete the survey will receive a $20 reward.

Data Helps Us

  • Identify Community Health Needs
  • Determine how to meet those needs
  • Find resources for the community

Data will be used to

  • Document health issues and priorities in the community
  • Identify important issues to research
  • Support development of new or existing health-related programming and services

APCA was formed in 2002 to meet the health related needs of Asian American, Native Hawaiian and Pacific Islander (AA and NHPI) individuals and families residing in Arizona. Our vision is to inspire diverse communities to seek healthier futures. Our mission is to provide services, advocacy and education for diverse communities resulting in a healthier and more empowered population seeking good health. Our current programs include Enrollment for Health Insurance such as Patient Protection and Affordable Care Act Marketplace (Obamacare), Medicaid (AHCCCS), and referrals to other programs. We are also working on improving oral health access for all Arizonans and ensuring everyone in our beautiful community gets counted for the Census.
